The Health Promotion Case Manager, working within a harm reduction model, will use an intensive case management model to work collaboratively with clients to decrease vulnerability and risk for acquiring HIV while increasing their resiliency and self efficacy through the development of a personalized resiliency plan. The Case Manager will work with individuals from one of the following identified key populations: gay and bi men and other men who have sex with men (OMSM); others who have additional risk factors such as recent diagnosis/treatment of sexually transmitted infection(s), being part of a serodiscordant couple, participating in the sex trade; experiencing mental health and/or substance use issues; homelessness or inadequate housing. Over a time-limited period of 6-8 visits per episode of care with a follow up at 3-6 months, the Case Manager conducts an intake, uses a standardized risk assessment process to address health status, financial/legal issues, housing status, transportation, social support, nutritional health and mental health/addictions, and develops a personalized resiliency plan. This position reports to the Health Promotion and Community Case Management Supervisor.
- Consults with the Supervisor on client and program issues and directions.
- Conducts intake through a defined intake process based on referral criteria established collaboratively with VCH/PHC.
- Conducts a detailed assessment of client needs using a standardized risk assessment process established by VCH/PHC.
- Develops a personalized resiliency plan with a template established by VCH/PHC utilizing motivational interviewing, trauma informed care, asset-focused care, and client-generated goals.
- Facilitates referrals and linkages to social supports and clinical teams including but not limited to counselling, income, housing, legal, food, education, support and immigration support, or employment training, linkage to primary care and/or, clinical case management provided by mainstream health services or linkage to mental health and addictions services.
- Provides outreach support based on the resiliency plan such as accompaniments or education.
- Implements and provides ongoing evaluation of the personalized resiliency plan.
- Provides transition and discharge planning for clients.
- Maintains thorough knowledge of eligibility criteria, application processes and appeals procedures for government and community programs/services.
- Liaises with service providers to ensure client access to services.
- Maintains current knowledge of resources and services.
- Networks with service providers and builds collaborative inter-agency partnerships.
- Refers clients to other AIDS Vancouver support services and/or other community services.
- Completes all necessary documentation for client files.
- Collects outcome indicators through compiling qualitative and quantitative statistics and reports related to client and/or program services.
- Stays informed on an expert level related to harm reduction practices.
- Delivers education to client base.
- Maintains database to track client referral and follow-up information.
- Stays up to date on developments in HIV/AIDS related issues.
- Maintains outcome measurement and evaluation plan systems.
- Participates on internal and external committees relevant to program services as directed by the Supervisor.
- Participates in developing and implementing department work plans.
- Participates in professional development opportunities as required.
- Attends meetings as called by the Executive Director and/or the Supervisor when scheduled during regular working hours.
- Complies with agency policies and procedures.
- Performs other related duties as required.
Education and Experience:
- Completion of a Bachelors’ degree in an Allied Health discipline such as Social Work, Clinical Psychology or Nursing.
- Two years HIV case management experience in a community setting is desirable.
- Demonstrated skills in program development and building partnerships among allied service providers.
- Direct experience working with populations vulnerable to HIV/AIDS such as First Nations, LGBTQ2S and Other Men who have Sex with Men, youth, women, refugee/immigrant, and substance use individuals.
- Direct experience in working with risk factors related to sexual health, mental health, substance use problems and/or homelessness or inadequate housing.
- Direct experience in harm reduction and health promotion work.
- Direct experience working in an outreach capacity in an urban setting.
- Direct experience working with cross-cultural practice.
- High degree of familiarity with government systems and community resources.
- As an equivalent degree, AIDS Vancouver will accept a combination of education, training and experience which includes a minimum of 10 years of direct client service in HIV/AIDS.
