Position Summary

Reporting to the Executive Director and as a member of the Leadership Team, the Case Management Program Manager is responsible for the overall management of client case management services. The Case Management Program Manager is responsible for the implementation of practice standards and processes, established in collaboration with Vancouver Coastal Health Authority, to support the provision of the service and for the collection of defined outcome measures in addition to directly supervising the staff associated with the Health Promotion and Community Case Management services and The Corner working within a harm reduction model. This position reports to the Executive Director.

 

Duties

  • Consults with the Executive Director and the VCH Regional HIV Program Director on program issues and directions.
  • Monitors the delivery of Health Promotion and Community Based Case Management services and provides comprehensive case consultation.
  • Provides support to the client services team.
  • Ensures the coordination of services and support programs systems.
  • Ensures the coordination of other services as assigned.
  • Develops appropriate models of service delivery.
  • Provides ongoing clinical supervision to all Health Promotion and Community Case Managers.
  • Establishes and/or implements systems to monitor and review standards of practice.
  • Supervises staff by performing duties such as assigning work, providing feedback and performance evaluation, determining training requirements, orienting new staff and maintaining timekeeping and attendance records.
  • Resolves staffing problems, including calling in staff to ensure appropriate staffing levels.
  • Provides direction and support to all assigned staff.
  • Participates in recruitment and selection of staff by performing duties such as screening applicants, participating on interview panels, providing input into the development of interview questions and making hiring recommendations; responsible for the retention, as required of assigned staff.
  • Coordinates, supervises and evaluates practicum students and volunteers assisting in the operations of client services.
  • Maintains client database in Penelope to track client referral and follow-up information.
  • Stays up to date on developments in HIV/AIDS related issues.
  • Assists with developing funding proposals.
  • Compiles qualitative and quantitative statistics and reports related to client and/or program services.
  • Participates in developing and implementing department work plans.
  • Maintains outcome measurement and evaluation plan systems.
  • Participates on internal and external committees relevant to client and/or program services as directed by the Executive Director.
  • Participates in professional development opportunities as required.
  • Attends all meetings as called by the Executive Director when scheduled during regular working hours.
  • Complies with agency policies and procedures.
  • May attend Board Meetings at the discretion of the Executive Director.
  • Performs other related duties as required.

 

Qualifications

 

Education and Experience:

  • Completion of a Masters’ degree in an Allied Health discipline such as Social Work, Clinical Psychology or Nursing.
  • Two years experience in a supervisory capacity.
  • Four years HIV case management experience in a community setting.
  • Direct experience in utilizing Penelope Case Management Database, or equivalent.
  • Demonstrated skills in supervision, program development and building partnerships among allied service providers.
  • Direct experience working with populations vulnerable to HIV/AIDS such as First Nations, LGBTQ2S and Other Men who have Sex with Men, youth, women, refugee/immigrant, and substance use individuals.
  • Direct experience in working with risk factors related to sexual health, mental health, substance use problems and/or homelessness or inadequate housing.
  • Direct experience in harm reduction work.
  • Direct experience working in an outreach capacity in an urban setting.
  • Direct experience working with cross-cultural practice.
  • High degree of familiarity with government systems and community resources.
