PHAC – Online Health Survey for Health Practitioners on HIV Screening & Testing

phacThe Public Health Agency of Canada is conducting an anonymous survey for health professionals to examine knowledge and practice related to HIV screening and testing. The survey will take about 15 minutes to complete, depending on your answers. You may receive up to four invitations to participate in this survey.

Your response to the survey is requested if:

  • You are currently practicing
  • You are a:
    o Physician (GP and specialist)
    o Registered nurse
    o Nurse practitioner
    o Counsellor
    o Social worker
    o Community health worker
    o Midwife
    o Community –based service provider
